Frailty training package
As part of the Ageing Well plans for the Integrated Care System (ICS) one of the areas of focus is the identification of frailty.
The Clinical Frailty Scale (CFS) provides a shared language between all areas, helping everyone to support older people to live well.
The first step is launching our new frailty training package.
This new frailty training package is for:
All people working in health and social care who support adults 65 and over across the Nottingham and Nottinghamshire ICS.
Why this training is required:
To introduce the Clinical Frailty Scale (CFS) and ensure that the whole health and social care workforce across the Nottingham and Nottinghamshire ICS can communicate using a common language and so improve the quality of life for people living with frailty.
All people having undertaken the training will understand:
- Frailty and the use of the Clinical Frailty Scale in the identification and communication of frailty.
- How to describe the deterioration or reablement of individuals.
- The importance of a holistic, joined up approach to managing frailty across health and social care.
- How to identify an appropriate care pathway for those living with frailty including referral and signposting to appropriate services.
