Make a HPAS referral as a professional

This service is for

Healthcare and social care professionals to refer people needing care or support to the HPAS service.

You will need

As well as providing your own details we will also need the person's:

• name, address, contact information and date of birth
• property ownership details
• details of the required work
• third party contact information if you or the person cannot arrange access with the tradesperson.

What will happen next

The referral will be reviewed in office hours and allocated to a tradesperson. If we need more information about the work required we will contact you (or the third party contact).

We aim to allocate the job and have it completed within:

• two working days - for hospital discharge referrals
• five working days - all other referrals.
