The form should be completed by the pupil/student or their parent(s)/carer(s). The information on the form will be used to decide if the pupil/student is entitled to special travel assistance and if they are, what form it will take.
You will have the opportunity to upload supporting documents to corroborate what you put on the form.
What will happen next
This is a short form which should only take a few minutes to complete.
If your application is approved, it will normally take 4-6 weeks to set up transport or make the arrangements for a personal budget.
