SEND Top-Up Funding Requests for Mainstream Schools

Please submit all requests for HLN funding through this online form. This includes requests to HLN Panel and exceptional HLN requests. You will find the panel dates for the year below and please share an ‘intention to submit’ notification for any funding requests you are preparing, to trigger requests for supporting reports from professionals involved. Families of Schools can also use this online form to prepare AFN requests, if they wish to, but this is not required.

This service is for

For most funding requests, the school where the young person is currently on roll should complete this online form. The form should usually be completed by the school SENCO or head teacher.

  • Attendance information 
  • Involved external agencies 
  • Academic Progress details 
  • Special Educational Needs, Barriers and Provisions 
  • Additional support documentation (this can be uploaded to the request)
