Submit evidence
If your evidence is collected and you know which of the four levels of accreditation you want your organisation to have for the Nottinghamshire Wellbeing at Work scheme, you can now submit this via this form.
Eligibility
All organisations - whether small, medium or large - are able and encouraged to get this accreditation.
You will need
- to know the award level that you are applying to
- the appropriate evidence strands for all four elements of the scheme, evidencing steps you have taken to prevent substance misuse, increase mental health and healthy eating and educate on protecting health (e.g. images of posters put up in the office)
- to know the % of staff off staff sickness absence in the past year - this can be an estimation.
What will happen next
You will be guided through the form and asked to submit the required level of evidence depending on the level of accreditation you want for your organisation.
A member of our team will then review your submission within 28 working days and either be in-touch directly or send you your certificate.
Please note: The form automatically times-out after an hour and you will not save automatically. Please 'save' as you complete the form if you're likely to take longer than this to submit all of the necessary evidence.
